How to Apply for CDCP

Check Eligibility

You’re eligible to apply for the CDCP if you’re a Canadian resident for tax purposes, you filed your tax return for the previous year, you don’t have access to dental insurance, and your adjusted family net income is under $90,000.

Apply Online

To apply for the CDCP, you’ll need the Social Insurance Number (SIN), date of birth, full name, home and mailing address, list of dental coverage through government programs (if applicable), and proof of tax filing for the previous year for yourself, your spouse/common-law partner, and your children under 18.

Complete Just One Application

You only need to submit one application for everyone who qualifies. This will help the process go faster and prevent delays.

Application Assistance

A trusted friend or family member can help you apply, or a delegate can complete your application for you.

Check Status

You can monitor the status of your application with a simple online tool. You’ll need your application code/client number and your SIN.
